The hemispherical Acetabular Component Press-Tit without Screw Fixation: Two-year Clinical and Radiographic Analysis

Abstract
The results of fifty-four consecutive primary total hip arthroplasties in which the acetabular reconstruction was performed using the hemispherical porous ingrowth acetabular component press-fit into the innominate bone without screw
fixation were reviewed after a minimum of two acetabular fractules. At last follow-up, no socket had been revised for loosening and none were radiographically loose. There was no acetabular lysis.
Areas of gaps beween the polar portion of acetabular component and the bone were present of the immediate post-operative radiograph in 19 hips. These gaps were located almost excusively near the polar portion of the implant. However,

percent of the gaps resolved or reduced by two years. On the final follow-up radiograph, progressive radiolucencies were identified two componcnts located in zone two.
These data showea the potential for acetabular fracture during insertion of pressfit acetabular components and the consideration of problems of screw augumented hemispherical cup fixation include frtting between the screws and metal
acetabular
components, inadvertent screw penetration into neurovascular structures, and tracking of polyethylene debris along the path of the screws, we recommend press-fitting without screw fixation for this acetabular component in prmary
cases
when the anatomy andthe bone stock permit.
